1. Routine Home Care
Private Home : Core service allowing the individual to stay at home
Nursing Home : Additional service to what is provided in the facility bringing expertise in pain assessment, pain control, and death and dying issues.
2. General Inpatient Care
Inpatient care at local hospitals for symptom management requiring 24 hour skilled nursing care unavailable in long term or private home settings.
3. Respite Care
24 hour care at local hospitals for up to 5 days to provide rest for caregivers.
4. Continuous Care
In the home : 24 hour care service for short time periods during crisis situations.

Who we are.

Autumn Wood Hospice Care was developed to provide dependable and timely hospice service, support and education specifically needed in the rural area. Hospice is a service with the passionate concept of caring for those who have life-limiting illnesses and/or are approaching death. Autumn Wood Hospice Care offers an interdisciplinary team that includes physicians, nurses, certified home health aides, volunteers, a social worker and chaplain to provide the skilled care and support required to meet the individual needs of each patient and his/her family.
